Open Day

QMSLSC are holding an Open Day on Saturday 20 September 2025 10am-2pm.

  • Great opportunity to find out more information about the Club
  • Members of the club will be available to help with additional information and the online registration process
  • There will also be information and sign up available for courses, patrols, board lease, youth activities, nipper activities
  • We will also be showcasing some of our lifesaving assets, checkout the JetSki (RWC), Rescue Boats (IRB's) and our eyes in the sky (Drone)

Family Membership Packages:

Family with 1 Child $360
Family with 2 Children $450
Family with 3 or more Children $540
  • Family Membership packages include up to 2 parents/guardians and children under 19 years (as at 30 September 2025)
  • All volunteers must be members
  • Children under 13 years (as at 30 September 2025) must join with at least 1 parent/guardian

Refund Administration Fees

Any refund of membership fees will be in accordance with the Club’s Membership Refund Policy: Membership Refund Policy

Identification

We will need to sight ID for each new member showing full name and date of birth.  Acceptable forms of ID are:

  • Birth certificate
  • Driver’s licence
  • Passport

Online Registration Process

Start the process of signing up by navigating to the SLS Hub Sign In page and selecting "Sign Up"

Decide on a Username and enter it into the “Username” field. Select "Next"

Enter your details as requested.

Note: If you are a parent signing up yourself and your family, the SLS Hub account details entered should be in your name

You will receive a verification code via email.  Enter the code and select "Verify Code"

Your SLS Hub account has now been created.

Once you’ve created your SLS Hub account, you will need to finish setting up your account by answering a few more questions.

Hover over the “I am a brand new member joining a club or other organisation for the first time” tile. Select “Proceed to Join Form”

Select “Quinns Mindarie” as the club you want to join

Individual Registration

Select “I am an individual looking to join a SLS Club” if the scenario applies to you. Select “Next”

Enter your personal details and select “Next”

Enter your contact and emergency contact details and select “Next”

Note: You are required to provide your parent/guardian details if you are under 18 years. They will be copied in on all email/SMS communication.

Tick to recognise and accept the Declaration. Select “Next”

Note: Your parent/guardian must acknowledge the membership declaration on your behalf if you are under 18 years

Select the applicable joining fees and select “Next”

Select the applicable add-on fees and select “Next”

View a summary of the fees you will pay

Select your desired payment method and enter the relevant details

If you are paying via Credit Card: Select “Pay and Submit Request”

If you are paying via Bank Transfer or Other: Select “Next”

Review the details of your request and select “Pay and Submit Request” (if you are paying via Credit Card) or “Submit Request” (if you are paying via Bank Transfer or Other).

Your membership application is now successfully submitted!

Once payment has been made, the club will process your membership request and send you an email when it has been approved.

Family Registration

Select “I am a parent/guardian (18+) looking to sign up my child/family” if the scenario applies to you.

Note: If you are looking to sign up your child/children only you must go back to the Sign In page and create an SLS Hub account for each child under their own name.

Note: If your child is under 13 years (as at 30 September 2025) they must join with at least 1 parent/guardian.

Select “I am looking to sign up myself and family members” if the scenario applies to you. Select “Next”

Enter the details of at least one primary member (Note: this must be the parent or guardian’s info) and the details of at least one standard member. Select “Next”

Enter your contact and emergency contact details and select “Next”

Note: Parent/guardian details are required for anyone under 18 years. They will be copied in on all email/SMS communication.

Tick to recognise and accept the Declaration. Select “Next”

Note: If one of the family members is under 18 years, the parent/guardian must acknowledge the membership declaration on their behalf

Select the applicable joining fees for each member (there is an option to select ‘included in Family Group’ for members who can be included in the Family Membership). Family Membership includes 1 or 2 parents/guardians and children under 19 years (as at 30 September 2025)

The Individual Joining Fee must be selected for any additional adults or children 19 years and over who aren’t included in the Family Membership Fee.

Select “Next”

Select the applicable add-on fees for each member and select “Next”

View a summary of the fees you will pay

Working with Children Check (WWCC)

If you are over 18 years and volunteer for the club, please provide your current Working with Children Check number and expiry date via email to info@qmslsc.com.au.

If you do not have a current WWCC card, please collect a form from the club office or the post office and complete it.

The club needs to complete and sign a section of the form – this can be any of the Board members or the Club Administrator.

This can be completed at the Open Day.

Please then take the form to the Post Office for processing.

The fee for WWCC for a volunteer is $11 which will be reimbursed by the club upon sight of the receipt.

Volunteer Declaration Form

If you are a parent volunteer (including nipper parents) who has a child registered as a member of the club and are seeking a volunteering exemption or you are a youth volunteer member (13 - 17 years of age) please complete the volunteer declaration form below.

Please note that the child declaration does not need to be completed on behalf of nipper members as they are not volunteers.
